The objective of this study was to evaluate reproductive traits in adults of Astyanax lacustris subjected to different spawning inducers. The study involved 240 females (12.54 g ± 2.33 and 7.66 cm ± 0.63 cm) and 240 males (5.83 g ± 0.39 g and 6.14 cm ± 0.64 cm), all at reproductive age. Three different inducers were evaluated: (i) 0.4 pellets of Ovopel®/kg of body weight; (ii) 0.5 ml of buserelin acetate/kg of body weight; and (iii) carp pituitary extract (CPE) (5.5 mg CPE/kg body weight for females and 2.5 mg CPE/kg body weight for males), as well as saline solution (without hormone). The degree-hours for spawning were greater (P<0.05) for the Ovopel® treatment (with 204.93) than in the treatment with CPE (183.2). Ovary weight and gonadosomatic index were higher (P<0.05) in CPE and Ovopel® treatments when compared to buserelin acetate. The number of oocytes per female, absolute and relative fecundity were greater (P<0.05) for Ovopel® and CPE treatments. Fertilization rate was higher (P<0.05) in treatment with buserelin acetate (82.3%) in relation to Ovopel® (72.33%) and CPE (62.40%) treatments, and the highest (P<0.05) hatching rates were achieved with buserelin acetate and Ovopel®. The number of larvae per female body weight was greater (P<0.05) when Ovopel® was used. In conclusion, Ovopel® proves to be a more effective reproductive inducer for induced reproduction of A. lacustris when compared to CPE and buserelin acetate.

Water stress affects plant performance at various organisational levels, from morphological to molecular, with a drastic drop in crop yield. Integrative studies involving transcriptomics and physiological data in recognized tolerant species are appropriate strategies to identify and understand molecular and functional processes related to water deficit tolerance. The cashew tree (Anacardium occidentale) is a species naturally adapted to environments with low water availability associated with adverse conditions such as heat, high radiation and salinity. We used an integrative strategy, combining classical physiological measurements with high throughput RNA-seq to understand the main adaptive mechanisms of cashew to water deficit followed by recovery. Physiological analyses indicate that young cashew plants display typical isohydric behaviour. They first exhibit rapid stomatal closure, followed by CO2 assimilation, thus preserving the relative water content, membrane integrity and photosystem II activity. Differential expression was observed in 1733 genes from plant leaves exposed to water deficit stress for 26 days. Among them, 705 were upregulated and 1028 were downregulated. After rewatering, 1330 (76.7%) genes returned to their basal expression level. Transcriptional, combined with physiological data, reveal that cashew plants display high phenotypic plasticity and resilience to acute water deficit, and do not activate senescence pathways. A series of genes/pathways and processes involved with drought tolerance in cashew are evidenced, particularly in carbon metabolism, photosynthesis and chloroplast homeostasis.

Sitophilus zeamais is a key pest of stored grains. Its control is made, usually, using synthetic insecticides, despite their negative impacts. Botanical insecticides with fumigant/repellent properties may offer an alternative solution. This work describes the effects of Anethum graveolens, Petroselinum crispum, Foeniculum vulgare and Cuminum cyminum essential oils (EOs) and (S)-carvone, cuminaldehyde, estragole and (+)-fenchone towards adults of S. zeamais. Acute toxicity was assessed by fumigation and topical application. Repellence was evaluated by an area preference bioassay and two-choice test, using maize grains. LC50 determined by fumigation ranged from 51.8 to 535.8 mg L-1 air, with (S)-carvone being the most active. LD50 values for topical applications varied from 23 to 128 µg per adult for (S)-carvone > cuminaldehyde > A. graveolens > C. cyminum > P. crispum. All EOs/standard compounds reduced significantly the percentage of insects attracted to maize grains (65-80%) in the two-choice repellence test, whereas in the area preference bioassay RD50 varied from 1.4 to 45.2 µg cm-2, with cuminaldehyde, (S)-carvone and estragole being strongly repellents. Petroselinum crispum EO and cuminaldehyde affected the nutritional parameters relative growth rate, efficiency conversion index of ingested food and antifeeding effect, displaying antinutritional effects toward S. zeamais. In addition, P. crispum and C. cyminum EOs, as well as cuminaldehyde, showed the highest acetylcholinesterase inhibitory activity in vitro (IC50 = 185, 235 and 214.5 µg mL-1, respectively). EOs/standard compounds exhibited acute toxicity, and some treatments showed antinutritional effects towards S. zeamais. Therefore, the tested plant products might be good candidates to be considered to prevent damages caused by this pest.

Background Complex percutaneous coronary intervention (PCI) is associated with higher ischemic risk, which can be mitigated by long-term dual antiplatelet therapy (DAPT). However, concomitant high bleeding risk (HBR) may be present, making it unclear whether short- or long-term DAPT should be prioritized. Objectives This study investigated the effects of ischemic (by PCI complexity) and bleeding (by PRECISE-DAPT [PRE dicting bleeding Complications in patients undergoing stent Implantation and Sub sequent Dual Anti Platelet Therapy] score) risks on clinical outcomes and on the impact of DAPT duration after coronary stenting. Methods Complex PCI was defined as ≥3 stents implanted and/or ≥3 lesions treated, bifurcation stenting and/or stent length >60 mm, and/or chronic total occlusion revascularization. Ischemic and bleeding outcomes in high (≥25) or non-high (<25) PRECISE-DAPT strata were evaluated based on randomly allocated duration of DAPT. Results Among 14,963 patients from 8 randomized trials, 3,118 underwent complex PCI and experienced a higher rate of ischemic, but not bleeding, events. Long-term DAPT in non-HBR patients reduced ischemic events in both complex (absolute risk difference: −3.86%; 95% confidence interval: −7.71 to +0.06) and noncomplex PCI strata (absolute risk difference: −1.14%; 95% confidence interval: −2.26 to −0.02), but not among HBR patients, regardless of complex PCI features. The bleeding risk according to the Thrombolysis In Myocardial Infarction scale was increased by long-term DAPT only in HBR patients, regardless of PCI complexity. Conclusions Patients who underwent complex PCI had a higher risk of ischemic events, but benefitted from long-term DAPT only if HBR features were not present. These data suggested that when concordant, bleeding, more than ischemic risk, should inform decision-making on the duration of DAPT. (AU)

Quality control (QC) in the pharmaceutical industry is a key activity in ensuring medicines have the required quality, safety and efficacy for their intended use. QC departments at pharmaceutical companies are responsible for all release testing of final products but also all incoming raw materials. Near-infrared spectroscopy (NIRS) and Raman spectroscopy are important techniques for fast and accurate identification and qualification of pharmaceutical samples. Tablets containing two different active pharmaceutical ingredients (API) [bisoprolol, hydrochlorothiazide] in different commercially available dosages were analysed using Raman- and NIR Spectroscopy. The goal was to define multivariate models based on each vibrational spectroscopy to discriminate between different dosages (identity) and predict their dosage (semi-quantitative). Furthermore the combination of spectroscopic techniques was investigated. Therefore, two different multiblock techniques based on PLS have been applied: multiblock PLS (MB-PLS) and sequential-orthogonalised PLS (SO-PLS). NIRS showed better results compared to Raman spectroscopy for both identification and quantitation. The multiblock techniques investigated showed that each spectroscopy contains information not present or captured with the other spectroscopic technique, thus demonstrating that there is a potential benefit in their combined use for both identification and quantitation purposes.

RESUMO

A prótese total sobre implantes é uma alternativa para pacientes totalmente edêntulos com rebordo ósseo que não proporciona retenção e nem estabilidade para uma prótese total convencional. A prótese implanto-suportada e implanto-retida é o tipo de prótese que os pacientes têm maior preferência, sendo uma das melhores formas de reabilitação para pacientes desdentados total, reestabelece a função mastigatória, deglutição e também a capacidade fonética, trazendo maior conforto e segurança para o paciente resultando em melhor qualidade de vida. O objetivo deste trabalho é mostrar uma técnica alternativa para reabilitação total superior com prótese total implanto-suportada cerâmica com infraestrutura de zircônia.


The total prosthetic implant is an alternative to edentulous patients with bone edge which does not provide retention and stability or for a conventional denture. The implant-supported and implant-retained prosthesis is the type of prosthesis that patients have more preferably, one of the best forms of rehabilitation for full edentulous patients, restores chewing, swallowing and also the phonetic capacity, providing greater comfort and safety to the patient resulting in improved quality of life. The objective of this work is to show an alternative technique for maxillary complete rehabilitation with implant-supported denture with ceramic zirconia infrastructure.

The performance of Nile tilapia (Oreochromis niloticus) raised in cages furnished with an automatic dispenser, supplied at different frequencies (once per hour and once every two hours) and periods (daytime, nighttime and both) was evaluated. Eighteen 1.0m³ cages were placed into a 2000m² pond, two meters deep with a 5 percent water exchange. One hundred and seventy tilapias, with initial weight of 16.0±4.9g, were dispersed into each 1m³ cage and the feed ration was adjusted every 21 days with biometry. Data was collected from March to July (autumn and winter). Significant difference to final weight (P<0.05) among treatments was observed. The increase in feeding frequency improves the productive performance of Nile tilapias in cages and permitted better management of the food. The better feed conversion rate for high feeding frequency (24 times day-1) can result in saving up to 360kg of food for each ton of fish produced, increasing the economic sustenance for tilapia culture and suggesting less environmental pollution.


Avaliou-se o desempenho de tilápias-do-nilo (Oreochromis niloticus) produzidas em tanque-rede, providas de dispensadores automáticos de ração, alimentadas em diferentes frequências - uma vez por hora e a cada duas horas - e períodos - durante o dia, à noite ou ambos. Dezoito tanques-rede de 1.0m³ foram colocados em um tanque de 2000m² com dois metros de profundidade e renovação de água de 5 por cento. Cento e setenta tilápias, com peso inicial de 16.0±4.9g foram distribuídas em cada tanque-rede de 1m³ e a taxa alimentar foi ajustada a cada 21 dias junto com as biometrias. As medidas foram coletadas de março a julho (outono e inverno). Observou-se diferença significativa para peso final (P<0.05) entre os tratamentos. O aumento da frequência alimentar melhorou o desempenho produtivo de tilápias-do-nilo produzidas em tanque-rede e permitiu melhor manejo alimentar. A melhor conversão alimentar para alta frequência, 24 vezes dia-1, pode resultar em uma economia de até 360kg de ração por tonelada de peixe produzido, melhorando a sustentabilidade econômica para produção de tilápia e sugerindo menor poluição ambiental.

OBJECTIVE: The purpose of this study was to discover the differences among victims who had traumatic brain injury due to traffic accidents. METHODS: Medical record of the head injury patients were analyzed according to their classification as traffic accident victims (pedestrian, motorcyclist or passenger and other motor vehicle deriver or passenger), age, gender, admission type (admitted from scene of the injury or from another hospital), duration of hospitalization, type of head injury, types of lesions present in other body segments and mortality. Patient's injury severity was measured by Injury Severity Score and head injury severity was analyzed using the ranking on the Glasgow Coma Scale, recorded by neurosurgeons during their first neuro assessment. All head injured patients admitted to a trauma center in S. Paulo city over a four-month period from March through June 1993, were included in the study. The sample was of 156 victims, with subsets of 80 pedestrians, 26 occupants of motorcycles and 50 occupants of other motor vehicles. RESULTS AND CONCLUSION: The results of this study showed that the mortality rate was higher in the pedestrian subset (25.0%) than among other victims and higher for motorcycle occupants (19.2%) than for motor vehicle victims (8.0%). Statistical differences between the subsets were established when the head injury severity variable was analyzed using the Glasgow Coma Scale. On the other hand, the differences between the three subsets was not statistically significant when the measurement used was the Injury Severity Score. Analyses of other variables showed important differences among subset distributions.

The study had the purpose to investigate aspects of the ICUs from the Municipal district of São Paulo, regarding the composition of the clinical staff; characteristics of the nursing personnel and nurses' characterization. Data were collected through 47 questionnaires distributed in 32 hospitals. Results show that: the morning and the afternoon shifts have a larger number of people from all clinical categories, the majority of the nursing personnel work 36 hours per week, nurses' wages varied from 700 to 1,500 "reais" and 66.1% of them were enrolled in Master Programs. Authors concluded that 79.2% of the clinical staff was formed by the nursing personnel.

A prospective and longitudinal study about head injury victims was carried out in order to identify their return to productivity after 6 months and 1 year post-trauma and to analyze the relationship between their return to work and their educational level as well as to determine the relationship between their return to work and their type of job before the onset of the injury. Of the 72 patients who were students or employees pre-injury, or housewives at the time of injury, the majority (73.6%) had returned to productivity 6 months after onset of injury. Of the victims who had returned to productivity, 25.0% had changed their original occupations or reported significant problems when returning to productivity. Baseline return was achieved in 48.6% of the victims at 6 months. Return to productivity was higher at 1 year after injury than at 6 months. When the relationship between return to work and educational level and job type was analyzed, return to work was not related to educational level or job type.

The proposal of this research was to obtain parameters to start or maintain cardiopulmonary resuscitation (CPR) in victims of trauma. The duration of the cardiac arrest and the CPR of the survivors was described, as well as the cerebral performance and the mortality of these victims 24, 48 and 72 hours after these events had happened. With the results of this characterization the relation between duration of cardiac arrest time, CPR and mortality were described. Data for this report were collected in Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo emergency department. A big amount of the victims (93.4%) presents severe trauma and main cause of death was brain injury. Survival at 72 hours after CPR was 10%. The assessment, during the 72 hour period, of the survivors from cardiac arrest of traumatic cause has shown bad cerebral performance of those victims in that period of time. The survivor after the first episode of CPR was strongly related to cardiac arrest time when compared with CPR time. The time of cardiac arrest < or = 4 minutes and CPR < or = 20 minutes was related to survival more than 72 hours.

This study is part of a project about ICU's characteristics in São Paulo city. This article describes the continuing education programs for nursing staff. 43 ICUs were analysed and the questionnaire answered by the ICU nurse coordinator was used to collect data. Results showed that 34 (79.1%) of the ICUs have initial program for training each nursing staff category and 18 (41.9%) had regular continuing education program focusing primarily on nursing procedures and routines and the update in pathologies. Continuing education programs are developed primarily by ICU's nurses. 50.2% of the nurses answered that they attended specialisation/extension courses too, in Medical Surgical Nursing or in other areas or in both areas. Due to this result and the development of the ICU's nurses as specialists, some suggestions are presented to improve the continuing education program in these Units.

AIM: To evaluate the use of local transdermic anesthetics in fine needle aspiration biopsy (FNAB) in breast lesions. METHODS: Prospective randomized study of 119 patients having breast lesions, all being indicated for FNAB. The patients were divided into three groups: 40 patients entered in the active group (lidocaine + prilocaine); 40 patients underwent the placebo group (aqueous extract of Triticum vulgaris); and a control group of 39 women in whom FNAB was performed without the administration of any substance. Both the anesthetic and placebo were administered an hour before FNAB. Pain was quantified through a visual analogic scale of pain. The type of pain was also classified in terms of occurrence: only during the puncture, only during the movements and both. RESULTS: The visual linear analogic scale of pain showed an average of 3.3 in the active group, 3.5 in the placebo and 4.0 in the control group (NS). Analysis of the type of pain which was referred by the patient showed that 15% of the patients in the active group, 12.5% of those in the placebo group and 5.1% in the control group did not refer to any sensation of pain. Pain, when felt, was similar in all three groups (p < 0.4). CONCLUSIONS: Both the quantification and the type of pain referred to were similar in all three groups. However, there was a tendency of the patient to refer to less pain when the active substance or the placebo were used, when results were compared to the control group.

RESUMO

This study was developed in order to compare the use of the ABBREVIATED INJURY SCALE (AIS) and the CONDENSED ABBREVIATED INJURY SCALE (CAIS) as basis to calculate INJURY SEVERITY SCORE (ISS) in head injured patients. The results showed that the ISS value was equivalent in the majority of the patients (58.51%) codified by both scales. Also no statistic differences between the scales were perceived when we compared the severity levels as severe, moderate and minor, 61.38% of the lesions scored by AIS/90 were scored by CAIS/85, too.

The Glasgow Coma Scale (GCS) and the Jouvet Coma Scale (JCS) have been evolved for assessing the depth and duration of impaired consciousness and coma. The analysis and the utilization of these scales have showed that they are complementary. The GCS is more sensitive when there is a more intense loss of consciousness, whereas the JCS shows its sensitivity better in the states close to normal. This study was aimed to compare the results obtained from the evaluation of the consciousness level by the utilization of the two scales. The comparison was done within a prospective study with 48 patients, all of them over 18 years old, interned in three intensive care units of different hospitals in the city of São Paulo. The evaluations were done daily by the researchers and the scales applied in sequence totaling 5 minutes. Each scale was applied in 106 evaluations, and the results showed a statistically meaningful difference between the GCS and the JCS as to the indication of alteration in the consciousness levels. In 37.74% of the evaluations done with the JCS there was an indication of alteration in the consciousness level, whereas with the GCS the alteration was present in only 23.58% of the evaluations. Another important observation about the utilization of both scales was that people whose scores were between 9 and 10 in the GCS had had an stronger indication of alteration of consciousness level by the same scale, while those with scores between 12 and 15 had a stronger indication of alteration in the consciousness level by JCS. When using GCS there has been the application of the non-testable (NT) in 20% of the evaluations. This did not occur when using the JCS. However it is believed that specific conditions of that particular group might have led to that result as well as specific characteristics of groups of patients might favor the utilization of different scales to evaluate the consciousness level. Therefore the final choice between such scales should consider the conditions and the peculiar characteristics of the clientele to be evaluated and not individual or health department services preferences.

A prospective longitudinal study of traumatic brain injury (TBI) patients was conducted to identify the recovery pattern 1 year after trauma. Patients with all levels of injury and age between 12 and 60 years were observed. They were analyzed using the eight-point Glasgow Outcome Scale (GOS) as well as their return to productivity in that period. The majority of victims (77.2%) made good recovery, that is (GOS = 0 or 1) and complete recovery or GOS 0 was achieved in 38.6% of then. However at the 1 year mark, 22.8% of the victims showed disabilities. Overall, victims had returned to productivity in that period (83.3%) but 19.4% of them have had changes in their productivity and 16.7% didn't return to your job.).

RESUMO

The goal of this study was to analyse some practicing, teaching and learning aspects of physical examination done by ICU's nurses. It was accomplished with 26 ICU nurses that concluded the Intensive Care Nursing Specialization Course at the School of Nursing at São Paulo University. The results showed that 31 (68.9%) of the 45 presented items were done frequently by more than 50% of the nurses. The professional practice was considered the most important moment to physical examination learning. The responsibility by teaching was attributed to undergraduation course by 69.2% of the nurses.

É apresentado um caso clínico de rectorragia devida a uma duplicaçäo gástrica cística numa criança. Para além da presença de heterotopia pancreática localizada ao cisto, existia também uma duplicaçäo pancreática; esta associaçäo de anomalias näo foi anteriormente descrita. As rectorragias foram causadas por erosäo da parede do cólon transverso, subseqüente à perfuraçäo do cisto. Uma aderência embrionária endo-ectodêrmica originou, provavelmente, a duplicaçäo gástrica com heterotopia pancreática, assim como a duplicaçäo pancreática por um mecanismo semelhante à formaçäo de um divertículo de traçäo

RESUMO

This is a case of a 5-month-old girl with massive rectal bleeding caused by a gastric duplication. Cyst heterotopic pancreas and pancreatic duplication, an association not previously reported were present. The peculiar clinical presentation was due to cyst perforation that had subsequently eroded the transverse colon. Coexistent gastric containing heterotopic pancreas and pancreatic duplications were probably produced as traction diverticular by an embryonic entoectodermal adhesions.

Atresia of the colon (AC) is an uncommon cause of neonatal intestinal obstruction requiring prompt surgery. However, its optimal operative management is matter of discussion. We report the surgical therapy and evolution of three cases successfully treated at our department. A type I case was submitted to a coloplasty plus resection of the diaphragm; two cases of type III underwent a two-staged procedure, i.e., colostomy with subsequent anastomosis without resection. After analysing the different operative procedures we suggest that a colostomy with later establishment of intestinal continuity may be an advantageous procedure even in the healthy patients with proximal lesions. A coloplasty is a safe alternative to type I atresias.
